Service Profile (Service Job Order)

This section is the Service Profile. This form contains the fields for creating a Service record for job ordering.

Overview: Service Profile states information about the job order. such as,

• JO number – system generated record number (JO03 is a service type)
• Date – date of registration as draft
• Project No. – reference project number of the job order
• Project Title – reference title of PIN
• Project Location – reference project site
• Add/View Map - provision for uploading image of the site
• PM – officer in charge of JO
• Status – status of job order; see status names
• Remarks – additional notes
• Task/Service – selection for the type of service/s to conduct
  • ins - installation/ ocu - ocular/ smt- site measurement/ mnt-maintenance/ oth-others
• Additional Info – additional task/service option not in selection
• Department – reference department

* Service Details Tab:

• Start Date – date when service will commence
• Time – schedule of service
• End Date – expected date of completion
• Contact person – name of client representative
• Contact no. – mobile number of client representative
• Service Location – address of service
• Special Instruction – detailed memo or specifications

* Manpower Requirement – tab for listing names of suggested personnel to implemented the intended service

• Lead installer – name for foreman/leader
• Date needed – same as start date
• Number of installers – total number of installers plus extras needed
• Employee Permit – updated list of active installers for selection

*Service report tab:

• Date Completed - Last day of rendering service
• Days Completed - number of days taken to render the requested service
• Report Employee - check list for reporting installers who rendered/participated during the service
• No. of Extra -ave. number of extra manpower used during the service
• Comment - written remark or suggestion pertaining to service experience
• SRF no. - reference service report no./s used for the entire service
• Additional Report Employee - name of employee who participate but not in checklist

Important Notes:

This Job order is used when given an order or schedule from client when to start installation or attend other special services such as site measurement, ocular, maintenance, and the like or as initiated by operations.
